The IRS has released final 1094C and 1095C forms and instructions for the 2017 reporting year. The forms have only minor changes thus far and can be accessed using the links below:
- Form 1094-C: Box C in line 22 has been removed and is now labeled as “Reserved”.
- Forms 1095-C: A paragraph has been added to the instructions which directs individuals to the IRS website to provide information on the individual and employer shared responsibility provisions and premium tax credits.
Please keep in mind the following:
- Forms 1094-C and 1095-C are completed by Applicable Large Employers (employers with 50+ full-time equivalent employees) that are both fully-insured and self-insured.
- Forms 1094-B and 1095-B are completed by employers with fewer than 50 full-time equivalent employees that have a self-funded medical plan. The final instructions for these forms are not yet released.
These forms will be completed in early 2018 for the 2017 calendar year.
